The health information you provide on this site is confidential. It will be available only to you and Avera clinical and project staff. Personal information will NOT be provided to anyone else without your written permission. Your participation is entirely voluntary and you are free to discontinue participation, including your authorization regarding the use and disclosure of your health information, at any time without prejudice to you or your medical care.

If serious questions arise regarding the safety of participants or confidentiality of personal data, personal information may be provided to the Office of Human Protections in the U.S. Department of Health and Human Services. No personal information, including, but not limited to, information collected on this assessment, will be provided to anyone else without your written permission (letter or e-mail).

The Breast Cancer Risk Assessment asks you to provide information about your health only for the purpose of generating a personally tailored description of your disease risk. It does not sell, rent or lease any of the information you supply.
